There is a Dose rang: Like most other pain pills, the dose can be adjusted upwards but it is best done under the supervision of your doctor who prescribed it for you.
The standard dose is 100 every 8 hourly(300mg). But the dose can be increased to a maximum amount of 400 mg over 24 hours, if the lower dose is not adequate for pain relief.

If it is breast cancer(I asssume) then  it will depend on extent of disease in the bones(how many bones are affected ) the extent of treatment(number of drugs already used up and what remains behind). Not your call: There are some ringers for metastatic disease. Don't self-diagnose based on something you saw on the internet. If you are having a scan done, I understand how difficult this time must be and you have my sympathy. But the report by the radiologist should be available by the time you get this.

Arthritis: Arthritis is the most common cause of ankle pain without an injury. Bone metastasis in the ankle is a relatively rare occurrence.

Accepted Answer

Different analgesic: Might be more prudent to choose another analgesic.  Metastatic bone disease is quite painful, but we’d still need an analgesic that’s effective while causing minimal/bearable side effects.

Very rarely: Low calcium is usually of no consequence and is asymptomatic.

Bone: Could be mets, but need thorough exam, possibly bone scan & work up.

do you recommend a g-tube on this patient.

Accepted Answer

Due to cancer: Platelets high can be a reactant to his metastatic disease, and his anemia can be due to his malignancy and bone metastases. I typically do not recommend a g-tube in advanced malignancy as this has never been proven to extend life, or improve quality. There is a higher risk of aspiration pneumonia, no improvement in weight, muscle mass continues to be lost despite the g-tube.

what are alternatives to zometa/xgeva for bone loss/sre prevention in breast cancer, bone mets that do not decrease calcium? thank you.

Not much else: The alternative is avoid both and not take anything as there is nothing else available that helps. But low calcium is of no consequence especailly with zometa.

78 yrd uncle has lung cancer bone mets now weakness in legs, blood transfusions 3 x pwk & referred to palliative team.  how long left approx.?

Difficult to say: I'm sorry to hear about your uncle. We try to refrain from "predicting" when an individual may succumb to an illness because truly there are too many factors coming into play.  That being said, usually palliative care referrals imply that the patient is not expected to survive more than 6-12 weeks more. We don't have crystal balls and frequently patients surprise us.
