Ear infections : The ear infection can definitely go on simultaneously with mono.  If your eardrum ruptures you will see blood and pus.  IIf your hearing is down you will need to seek treatment ( hearing test and antibiotic ear drops)

Scratch: The likely source of the blood is abrasion to the ear canal wall caused by the q-tip, if not at this time, then previously. In general, we advise you never use a q-tip or any solid device in your ear as it is a blind procedure and you're likely to impact the wax (push it against the drum). Liquids are okay like warmed oil, hydrogen peroxide, or cerumenex. Doctors use an otoscope or mirror to see.

To diagnose this, a doctor would need to look into your ear canal with an otoscope and see if there is any drainage in the ear canal. You would then be treated with antibiotic ear drops, if present, and perhaps an oral antibiotic depending on how it responds to the ear drops. So, see a doctor who can look into your ears.
