See a doctor: I would recommend seeing a gastroenterologist or colorectal or general surgeon. May be able to use suppositories. But alsoay need a colonoscopy, stool studies, etc. Hope this helps!

Hemorrhoids: Although hemorrhoids is the most common, many other more serious causes exist. You don't mention your age or sex. Could be colitis, anal fissure, colon tumor among other things. Remember: no amount of rectal bleeding is normal or should be ignored. See your doctor and possibly a colorectal surgeon for a proper evaluation.

Hematochezia.: If you are not having pain or urgency and the drops are at the end of the stool and not mixed in with the stool then this is likely due to hemorrhoids. Otherwise see your doc for a better history and exam.

SEE A DOCTOR: I am sensitive to this situation.  My mother had blood in her stool, diagnosed by her doctor as "hemorrhoids."  She died within years of complications from surgery for bowel cancer.  Get it checked out.  Your age is not listed--if you are young, there is probably another reason.

Blood on stool: Rare  blood with a very hard BM may occur as it may be irritation or hemorrhoids but if this is occurring over months you needs to have a colonoscopy and be evaluated . F/u/ with gastroenterologist, make sure not polyps , diverticuli, autoimmune disease, rarely mass

Anal fissure: I'm unclear as to why your anal fissure won't heal. I recommend consulting a proctologist to make sure of the diagnosis as well as the cause of this persistent anal problem. You will obtain expert treatment and follow-up of anorectal problems. A colorectal surgeon is a second choice, but in my experience a proctologist will spend more time on your problem as this is the area of his/her expertise

Metformin: Or glucophage is well known to cause gastrointestinal side effects, specifically diarrhea and while it seems to have helped with your diabetes, it might have caused you gi issues. Best would be to share with your treating doc as adjustments may be needed to your regimen. Losing weight, if applicable, will help with your diabetes regardless.
