Atopic Dermatitis: Are they very small (pin head) and itchy?  If so it could be contact dermatitis which would recur for 5 years particularly when exposed to the offending agent.  Hot water typically makes it feel better in the moment but worse in the big picture.  Treatment - steroid cream and limit exposure to offending agent (plants, cleaning agents, soaps are among many possibilities).

The soles of your feet also can be affected. Treatment is with super potent topical  prescription steroids.

Because rashes are difficult to describe in words, and even harder to visualize from words, his primary care doctor or a dermatologist can look at the blisters to help decide what they are.

Not definite, but...: Snapping fingers can be a nervous habit, and normal.  Wearing an unneeded neck brace  seems odd, possibly delusional.  Truly hearing voices is an auditory hallucination.  In fact this CAN be a normal part of grief or separation, such as "hearing" your dead grandmother.  Unfamiliar voices making assertions is common in psychotic disorders- depression  and schizophrenia. It's possible he needs help.

Eczema, allergies: suggest see dermatologist for eval and tx....could be eczema, contact dermatitis, or even an infection.
