Means need to: maintain good hygiene  ( of ears )  , to clean ear canal  regularly after shower with  Q tips , some times may need ear drops to soften the wax . that will enable him to remove easily

Qtips push wax in...: Never use Qtips  to "clean the ear" as this actually pushes the earwax down deeper into the ear canal potentially against the eardrum which as you have discovered is extremely uncomfortable. This dried compacted earwax pressing against the eardrum will require an ENT or experienced family doctor to mechanically remove the earwax, then inspect the eardrum for possible injury. Don't delay. Go now.

Scratch: The likely source of the blood is abrasion to the ear canal wall caused by the q-tip, if not at this time, then previously. In general, we advise you never use a q-tip or any solid device in your ear as it is a blind procedure and you're likely to impact the wax (push it against the drum). Liquids are okay like warmed oil, hydrogen peroxide, or cerumenex. Doctors use an otoscope or mirror to see.

To have paint in your ears with your symptoms - it suggests that you may have taken in a fair amount. Hearing loss: Get this checked out. You could have a ruptured ear drum. The black wax could be dried blood settling into the wax. That could be why your hearing aids aren't working.
