Take in evening : Thyroid meds are to be taken on empty stomach. Try to take ranitidine with evening meal and go on anti reflux diet.

No: There is possibly an increase in pneumonia in hospitalized patients taking strong acid reducers. No worry with tums (calcium carbonate).

Yes : It is ok to take additional antacid medication however i would let your doctor know that the prescription strength medicine is not working completely.

Need PCP: You need to establish with a primary care physician that can coordinate further need for tests vs med adjustments and can work with you until symptoms are contained and a clear diagnosis is formulated.

Yes: If you have Insulin resistance or even diabetes metformin is fine to take.  Some people have lost weight on metformin, perhaps due to GI side effects.  I haven't heard that it can interfere with diet pills but those can be dangerous on their own.

Prilocec OTC: Prilosec otc is the best  for acid reflux but be sure to get evaluate by a doctor.

Work well. : Most over the counter medications for acid reflux work well. I have had success in patients with tagamet (cimetidine). Take it as directed. Sit up for at least 2 hours after eating. Do not lay down. Avoid caffein such as tea or coffee, avoid chocolate and other foods that can aggravate it including spicy or hot foods. If it continues, see your physician who can provide a further work up and treatment.

Suggestions:: 1. Lifestyle changes: smaller meals. Less large meals (buffets, eg), alcohol and caffeine and spicy foods. Don't eat right before bed time.

2.  Medicines: h2 blockers work fairly well (ranitidine, famotidine). Ppis are a little better (omeprazole and esomeprazole). All are available as otcs. Presctiption drugs are good too..
