Environmental mods &amp;: self rewards.  Focus is better for everyone if distractions in the enviroment can be removed, so location, background noise, etc. all have a big impact on focus.  Break down tasks into small goals, set time limits, and then reward self when goal is met, i.e., make 5 calls in 20 minutes and take 5 for a quick break.  Make lists and check off when complete. Lemon, mint: Citrus scents tend to be uplifting and a few clinical trials indicate that lemon, in particular may help with learning.  Mint is also invigorating.

Unusual: Unusual for the benefits to diminish in a few days - however I have seen patients who require as much as 90 mgm/day - if effects continue to diminish need to change meds.

Depression: Cortisol imbalance can manifest in mood swings, fatigue, sleep issues, etc.  Consider seeing a nutritionally oriented doc or a nutritionist.  A blood test called Micronutrient panel can be useful.  See www.spectracell.com for more info.  See "Feeling Good"  which is a book by David Burns for some cognitive therapy self help.  See doc to rule out medical conditions and then an mental health pro.

It isn't definitive: When I have to study, I am often tempted to do something more pleasurable. Although this can occur in people with adhd, it could also  occur in just about anybody.  You have to supply the motivation to complete a task. If you are motivated, stimulants can improve focus.

Useful drug: Modafanil is indicated to treat daytime sleepiness associated with various sleep disorders, and may enhance both focus and concentration by increasing level of alertness.  However, doesn't make anyone more intelligent, and have yet to see it create a "rocket scientist".  But very safe and usually tolerated well, .

my doctor prescribed me 200mg tabs of modafinil. i took the 1st dose and felt nothing for hoursno noticeable increase in concentration/mood/motivation/alertness, etc. how long does it take for these effects to start? is something wrong? thank you?

: It often takes more than 1 dose for a medication to show the desired effects. So, continue to take the medicine as prescribed and if, after a week or so it is still ineffective, then call the doctor who is prescribing the medicine to discuss the effectiveness and alternatives if the medicine is not having the desired effect. we decreased pristiq (desvenlafaxine) to every other day since it caused apathy.now my vyvanse is less effective.it dn help my fatigue, motivation, focus, ect.relation?

What's being treated: Pristiq (desvenlafaxine) is essentially Effexor only not available generically. Vyvanse suggests you're being treated for either adhd or some sort of fatigue syndrome. I've never heard of Pristiq (desvenlafaxine)  causing apathy. I'm not your doc, but it sounds like maybe the antidepressant should have been increased rather than decreased. Talk to your doctor about your depression symptoms before blaming them on pristiq (desvenlafaxine).

A few possibilities: Depression is possible, hypothyroidism also and even autoimmune disorders like fibromyalgia. All can lead to fatigue, low libido, tiredness, foggy brain and low energy and motivation. Cold hands and feet are more likely along with fogginess  to suggest medical problems. This is only a short list. A thorough medical and possibly psychiatric evaluation are in order.
