It depends upon: It depends upon the time of the month of your menstrual cycle.  Very high risk for pregnancy if you are ovulating.  The pull out method is not a reliable form of birth control as a pre cum has viable sperm. At a minimum consider using condoms as a barrier to prevent pregnancy and STD transmission. Consider choosing a reliable form of contraception such as birth control pills, injection or IUD

Difficult: The problem with being overweight is that your fat cells secrete a hormone that your body confuses with estrogen. Your estrogen and Progesterone ratios determine when you menstrate and this determines your fertility. With the extra "estrogen" floating around your menses gets irregular, and your fertility. So it's not impossible to get pregnant but more difficult than for some. Metformin may help.

Not really: However,  not impossible.
 Sperm can stay around/survive in the female genital tract for fertilization for about 5 days or so, hence conception can still occur with intercourse on Day 10-11.

Days 12-17: Figure what day you expect your next period.  Count back 17 days from there, and have intercourse daily for the next 5 days.

Same chance: As if you had sex one time before ovulation. It depends on your age and your cycle, not the number of tries. Is 49 years your correct age? If so the chances are basically zero, i'm sorry to say. If this is not your correct age you might give your real age instead for a better answer.

Unlikely : I'm not sure what leads you to think that you are ovulating on day 7 of a 27 day cycle.The length of time it takes for an egg to mature can vary but your period usually starts 14 days after ovulation. There are luteal phase defects in which case this is shorter and there are fertility problems but I know of nothing that would make it longer.Regular 27 day cycles sounds like normal ovulatory cycles

Not directly: Dysmenorrhea is a symptom, not a disease, and so won't directly affect your fertility. Dysmenorrhea can be caused by problems such as endometriosis or uterine fibroids, which might affect conception chances. See a doctor for an exam to get a better idea.

Regular Sex: healthy balanced lifestyle and regular sexual activity at least 3 times a week specially during fertile part of your cycle will increase chance of conception. Having a normal menstrual cycle and normal sperm count is important. History of sexually transmitted disease will decrease your chances and increase your risk of ectopic pregnancy. Please take your prenatal vitamins.

ESRD and fertility: While being on dialysis does decrease fertility, pregnancy is still possible. Men are affected to a much much smaller degree then women. Many organizations recommend pregnant dialysis patients receive  more frequent dialysis than other patients in attempts to have healthier newborns. Talk to your doctor and dialysis team if your intentions are pregnancy. Good luck.
