Sometimes when  the first few teeth erupt it is hard to see how everything will match up and fit together. Yes: Infants can have their first teeth present at birth or as late as 15 months. The later erupting group often get several erupting at one time.It is not an indicator of growth or nutrition problems but a variation of normal. Most have a full set of baby teeth by 3y.

This is variable: Teeth eruption is quite variable in normal babies. The average quoted is bottom two by 4-6 mo & 1/mo untill all 20 come in. I have seen the bottom two present at birth or not in untill 15 mo. The top 4 are lined up ready to come down next  but again eruption is variable. Occasionally i see outer upper teeth come down before the center 4.

Probably decay: Your child is in pain because of infected teeth. Take him/her to a Dentist, preferably a Pedodontist (pediatric dental specialist) immediately. The American Dental Association &amp; American Academy of Pediatric Dentistry recommend that all children have 1st Dental appointment at Age 1 just to prevent scenarios such as this. Your Pediatrician or your General Dentist can help you find a Pedodontist.

Not particularly: Given that we study everything you will find charts &amp; average emergence patterns but that stresses "average". Dental eruption timing and sequence (location) is variable in infants. I'm just happy that I see them eventually. One/mo starting at 4-6m is the average.

Yes, babies can be: born with teeth, and some infants do cut their first teeth by 3-4 months, although 6-9 months is more common. At 16 w, infants start bringing things to their mouths and like to mouth things as part of their normal development, and it is not necessarily a sign of teething.  Before the lower incisors erupt, the surface of the lower jaw will flatten and if it is still narrow, teething is not imminent

Something else: It is possible that your baby is teething, but this is one the young side.  Signs of teething would be drooling and a bump on the gum tissue and some crankiness but typically not severe pain. Call the pediatrician asap. They can help you.
