Million $ question: Drugs, alcohol, high blood pressure, and sometimes no good reason.   The best prevention is good prenatal care, attention to diet, sleep and stress, managing any underlying illnesses beforehand.   Avoid smoke, exposure to infections.  Take good care of the mother!

Depends on Healthy: 3 wks and above are not even classified as early. 
10 weeks early and above the outcomes are good with lower risks of prolonged ventilator needs and can generally go home within a month of due date.
Every 2 weeks earlier, the risks of deathbad outsomes  go up and length of stay goes up.
Edge of viability (not counting good outcome) is 16-17 weeks early.

Accepted Answer

Several things: It depends on what stage a pregnancy is in & what the cause of early labor is. Options include hormones, a circlage (ring that goes around the cervix to hold it closed), strict bedrest, and drugs like beta-agonists and IV magnesium. Blood pressure meds & antibiotics are used for certain types of preterm labor. Steroids are used to help mature the baby's lungs & antibiotics to prevent infection.

No problem!: A normal 7 month-old does not need to drink water. Babies should be offered breast milk, or formula, when thirsty. Sometimes juice is used to help resolve constipation. Otherwise, juice is not needed. During periods of the stomach flu, pedialyte may be tried if a baby is not tolerating breast milk or formula. But, in general, babies do not need to drink water.

Many causes: Babies can deliver before term due to many causes.  Illness in the mother, teen pregnancy, short interval between pregnancies, smoking, maternal trauma, structural problems with the cervix or uterus, alcohol use during pregnancy, maternal diabetes, other chronic illnesses.  About 50 percent of the time no cause is apparent.

There are risks: Assisted reproductive technology increases the risk for prematurity and low birth weight (as well as other factors) mainly because of the  higher rate of twins and triplets etc.  My neonatology colleagues are kept quite bust caring for very small babies from reproductive technology. There may also be an increased risk for birth defects due to so called epigenetic effects.

Chromosomal: Differential is very wide ranging from genetic abnormality to family history to medication effect , drug / alcohol history, moms medical history , idiopathic ( cause unknown ).

From what ?: If the issue is your casual or regular use of any drug, there is no "Clean out". Abstinence may clear your system enough that a urine screen would be negative. However, if you are on the Child protective service radar &amp; they want me to check the baby for drug exposure I will find at least trace amounts used after the 1st trimester.  Be honest with your OBGYN and you may avoid issues.

Confusing question: Your wording does not give a clear picture of what information you want. Twins can be delivered vaginally or by c section. Conditions at the time of labor including baby size, presentation (head or bottom first), & urgency all may require surgical delivery.

Avoid: 7 month old babies are basically on breast milk or formula and starting solids slowly. Keep in mind that you are giving them the building blocks of nutrition and they are learning about food, consistency and flavor.  Natural home made veggies and fruit have plenty of flavor with out adding anything else.
