Bone pulled off: An avulsion fracture is typically a small break in the bone that is caused by the pull of a ligament or tendon that is attached to the bone. This is usually because of the speed of the injury or the quality of the bone in the area. One example is with an ankle sprain in which the inside part (deltoid ligament) pulls a piece of the bone off rather tearing the ligament.

Yes: Ligaments and tendons are soft tissue structures that insert on various bones.  When you pull them hard enough, either it will tear or it will pull a piece of bone off.  When the ligament or tendon pulls hard enough to pull bone off, it is called an avulsion.  Recovery depends on the area of body involved.  Some require surgery, others heal well on their own.

Avulsion fracture: Tibial spine fractures are characterized by fractures at the insertion site of the anterior cruciate ligament (acl) on the tibia and were first classified into 3 types. For displaced (type ii/iii) fractures, surgical treatment has included fixation with screws, sutures, or wires performed through either open or arthroscopic approaches. Optimal treatment methods remain controversial.

10 weeks ago i was diagnose with  a 5th metatarsal avulsion fx. my most recent xrays still show lucency. can a healed fx still show lucency?

As long as you do: Not have a jones fracture fifth metatarsal surguries usually heal very well. Make sure your vitamin d levels are up to snuff. 
I would be more concerned how the foot feels then a radiographic finding.
