Bruise with hematoma: The shin area is a very common location for a toddler to have a bruise from a fall.  Sometimes, a small vein near the area of injury is injured as well causing a small hematoma or "lump."  It should gradually get smaller over the course of a few weeks to months, but should no longer be painful after about 1 week.  It should not affect his walking.  If other symptoms develop, he should see his doc.

Install window guards on windows above the first floor (unless this is against your local fire codes.) make sure windows are closed or have screens that cannot be pushed out. Don't underestimate your child's ability to climb.

Bed time routine: Does the toddler have a regular bed time routine?  Does he fall asleep in his bed?  Establish a regular bedtime routine, which may include a bath, reading a book, and falling asleep in his bed without a bottle or sippy cup.  Toddlers and babies do best by falling asleep where they will be sleeping otherwise this can have them wake often.  Consider teething, molars may be coming in,  try motrin.

Can you?: You may very well be the motherly type. Being this young does not make you any less of a capable mom. However, do you have any feelings about what your other 18 yr old friends are doing? I'm sure by now you know how life changes after a child. Having a strong support system helps. So please think this through.

Call his Dr.: Call his dr. As an evaluation is the best thing to do for an 18mo old with a fever.  The dr. May recommend a medication to give him such as pediatric tylenol (acetaminophen) (acetominophen) to bring his fever down, and wold know the dosage to tell you based on your child's weight, etc.  Best wishes.
