i am 55 with pmb.  endometrial thickness on us was 9mm.  endo biopsy was done showing atrophic endometrium.  lining is supposed to be less than 4mm.  what is the next step?

Hysteroscopy: May be considered. The ultrasound findings can be caused by an endometrial polyp (which can bleed). The atrophic endometrium findings can be seen when biopsy did not include the polyp or area of thickening on the ultrasound.

Pathology 101: An endometrial biopsy is usually done to look for cancer, and the pathologist here saw no signs of cancer (malignancy/atypia) in the sample obtained.  The other findings can be seen with or close to menopause (natural or induced by surgery or medications), and can be normal in many situations, but context (why the test was done) is important to be able to interpret this result uniquely for you.

Normal: After menopause, your estrogen levels decrease.  Estrogen helps to grow the uterine lining before menopause.  Atrophy means the lining is thin because of low hormone levels.  These are all normal changes in the uterus due to menopause.

2 opinions on estradiol cream for vaginal atrophy. gyne prescribed it 3x a week and said risk of uterine cancer very low. my physicians assistant at my gp said i must take oral progesterone with it or my endometrium will thicken. who is right?

: Gradual bloodstream levels of estrogen by strictly vaginal use of estrogen sources , and can cause proliferation of endometrial cells.  Long term use may be managed by your prescribing physician by occasional ultrasound imaging of the endometrial lining. If the lining is equal to one cm. in depth, a biopsy may be indicated.

Irregular Endometriu: It could mean many things.  A polyp, fibroid, hyperplasia.  (this are benign).  At your age, cancer would be rare.  Your gynecologist will investigate this and let you know what the appropriate treatment is.

Proliferative phase: In a postmenopausal woman it means it is too thick, and tissue sampling (a biopsy) should be done.
In a woman with regular menstrual cycles it indicates that she is in the first half of her menstrual cycle.

2nd phase absent: There are two phases to the endometrium. The first half of the cycle the endometrium grows under the influence of estrogen only= proliferative phase. The second half of the cycle Progesterone is added if ovulation occurs= secretory phase. By definition on your report the endometrium was not secretory.
