An internist or family doctor; a geriatrician generally sees individuals that are frail or have complex medical issues. Therefore, some geriatricians may see younger adults too  that have multiple medical problems that affect their function(like those with MS or als).

Leave it up to child: And physician.18-years is usual cut-off age.Many children & their pediatrician want to maintain their relationship until child graduates college at age 22+ years. Adolescent pediatricians often fill the gap between pediatrician and internist or family dr. Some children want to leave their pediatrician by age 16, especially if they don't have a same sex dr.. Those kids should be allowed to graduate.

Yes, doctor can exam: A person with chest pain symptoms usually has to go to his doctor (or even to the Emergency Room) to be checked right away for a heart problems, UNLESS there is another convincing reason for chest pain (decided upon by talking with one's doctor). That means not waiting at home to see if it turns into a more serious condition. As the saying goes, "better safe than sorry."

There are some adolescent doctor specialists as well.  Some children with disease needing frequent visions will stick to their pediatrician for a long time as they know each other well.  So there are various answers and no absolute time.
