Congenital: Many cultures have an increased risk of various diseases due to marriage within the family.  The more such sharing of the gene pool occurs with closely related people the more of a chance that the gene expression will become dominant.  This is also true in some religions such as Judaism.

Palpitations: These are caused by many things. Stress, excitement, irritants both physical and chemical, pressure like a hiatal hernia or esophageal push or pull, and primary heart things, the causes are very many in number. Don't worry unless your doc tells you worry is appropriate. Most times these flutters are benign.

: Klebsiella is not an unusual bacteria but will probably require a different antibiotic for treatment. It has probably become more dominant because previous treatments have suppressed other bacteria (commonly E. coli).

The largest group are disorders in which there is clearly a genetic component, but non-genetic factors are also important. No easy answer.
