Gastroenterologist: An anal fistula can occur in inflammatory bowel disease, specifically Crohn's disease. Your other symptoms could be somewhat related or there could be gluten intolerance. I highly recommend an evaluation by a gastroenterologist. Appropriate tests will be done. Referral may be made to a colorectal surgeon to treat the fistula.

that all happened 1 month after my anal fistula surgery, i had problem urinating, and i just finished my course of levofloxacin urinating problem is %90 gone now but i've developed a pain in my urethra that i didn't have what could be the cause?

: Did a urine culture&amp; colony count document the presence of a urinary tract infection? If so, was a follow-up urine culture done&amp; was it negative? If so, your urethral discomfort could be from a nonbacterial urethritis or a prostatitis. Consult a urologist for evaluation &amp; treatment if urethral pain persists.  Appropriate testing will be ordered

i have a pre-existing complex peri-anal fistula that sometimes requires surgery for drainage. i could have an abscess or i could have covid 19. problem is i cant go to my surgeon and possibly contaminate everyone if im positive or i can get sepsis?

If having perirectal: pain, then get seen in urgent care/ER. If worsening, respiratory symptoms, getting sicker, then may need treatment, regardless of the source. Sepsis, has to be treated as it will get much worse.

Sure: Exercise is unlikely to change the course of an anal fistula. If you have an anal fistula, you should have it corrected by a board certified colorectal surgeon, but exercising is not likely to make your situation any worse (though the fistula may make the exercise uncomfortable).
