Like building a house, you can't paint the walls until they are up. Babies make sa in the last months of pregnancy and it goes up in the lining of the lung air sacs.Docs have some artificial sa that can be placed in babies lungs & help, but baby must begin making it.

Surfactant reverses the surfactant deficient state, but does not instantly "cure" rds but permits the use of less use of the ventilator and supplementation oxygen.  Surfactant use reduces pulmonary airleaks and reduces mortality in multiple randomized controlled clinical trials.

The typical manifestation of this is bleeding disorder, due to the inability of platelets to become activated.
