Not possible: It is possible for the child to be rH negative if both parents are hybrid rH positive (one positive and one negative rH gene), but if they are both type O can only have type O children. Either one of them is not the biologic parent, or someone's blood typing is a mistake. Someone had to give that B gene.

Yes: The rh factor is inherited through separate genetic means from the blood type. The rh factor is a dominant gene, meaning only one gene is needed to cause expression of the factor. So two rh + parents can have a rh+/rh- gene pattern and have a 1/4 chance with every conception of having an rh-/rh- baby, 1/4 chance of rh+/rh+ baby and 1/2 of carrier rh+/rh-.

Yes, : Yes, two parents can be rh- positive and have an rh- negative child.  When it comes to the rh system, each parents genotype is made up of two parts. A parent could be rh -- (2 negatives),  rh ++ (two positives) or rh +-.  The rh positive is dominant over the rh negative.  Therefore someone who has an rh + genotype could be either rh +- or rh ++.  So if both parents were rh+- they would each be considered rh+ genotype.  If each of those parent's contributed a (-) negative to  the fetus, the baby would be rh-- and thus she would have an rh negative blood type. 
I know this can seem a little complicated.  Hope it helps to take some of the apprehension away.

Blood typing: The 4 major blood groups are a, b, ab and o.  If your mother is group o, than neither of her parents could be type ab.  The others would be possible.  Likewise, being rh positive means that at least one of your mother's parents were rh positive, but not necessarily both.
