: All laboratory results need to be interpreted in the clinical context and the doctor who ordered the tests is usually in the best position to do that. Talk to the doctor who ordered the tests. Having said that, what you described does not add up. There are no valid tests on dried urine.

Evaluation: See a psychiatrist for an evaluation to be diagnosed correctly.  Labwork may be ordered to rule out any underlying illnesses which can mimic your symptoms. Once diagnosed, then it is time to discuss treatment options. There are several treatment options such as medication, journaling and cognitive behavioral therapy. You have to find what works for you.

Best 4 You or 4 Me?: It sounds like you are asking for antidepressants that would treat someone diagnosed w/ all three - w/ ocd, anxiety ; depression. But even if you were asking for ones recommended for each of these conditions separately, keep in mind that what is "best" for one person may not be "best" for another.  It might be a bit like asking about the best brand of jeans for people in brookline who are age 37.

Alternative: These meds. have the possibility of interacting with each other (although this can be minimized by starting with a low dose).  That said, there are safer alternatives. Personally, I tend to try Pristiq (desvenlafaxine) + Wellbutrin XL for patients who aren't doing well on an SSRI alone. But this is a matter of prescriber preference and what your physician feels is the most appropriate for you. Hope that helps!

Please get: Psychological treatment for the ocd you mention in your history.  If you allow this to continue you may find that you become unable to leave your house at all, which is called agoraphobia.  If you can't find a therapist, try to find a book about it and work with that.  Peace and good health.

YES: Please consult your PCP and have medication prescribed or adjusted. I would also recommend psychotherapy ( CBT-cognitive behavioral therapy ) . I would also suggest that you read The Relaxation Response by Herbert Benson, M.D., practice yoga, aromatherapy, massage therapy, painting, coloring or learn a new instrument: the brain has its' own natural calming chemicals, endorphins.

would 40 mg of cymbalta (duloxetine) generally be considered to low a dose for ocd/depression/anxiety ..i am contemplating going to 60 with my dr's approval.

Talk to your doctor: If you're tolerating the 40 mg dose, and your symptoms are not controlled, then your doctor can titrate your dose to 60 mg.
