Arthritis: Degenerative changes in the spine which seem chronic. I am assuming this is an MRI finding which does not necessarily correlate with back pain or predict the need for surgery in the future. Protect your back.

: It means you need to look at the x-rays with your Dr and have them explain exactly what is going on. Different Drs will used differnt terms for the same thing.  eBe certain you have told him all the symptoms you are having. Your back is talking to you it seems. Listen and take good notes, share with your Drs and you all will figure it out. .

PT: Physical therapy is the traditional approach. You should also get a good physical and comprehensive blood work. If it gets worse you will need some type of surgery.

Yes: Normally the vertebrae line up when going from one to the next.  An anterilisthesis simply means one varrtebrae is shifted forward on the other.  The facet joints are joints in the back of the spine and at the two lower levels you have developed some degenerative arthritic changes.

Depends: Most disks get better within 6 months with pt and conservative treatment, assuming that the disk is even related to the pain.  Keep an eye on spondy to make sure it's stable.  If you develop instability surgery may be in order.

: If you are chronically symptomatic and problem has not improved with aggressive physical therapy +/- local therapy, you need to see a spine orthopedist for laminectomy or a minimal invasive spine surgery with vertiflex procedure for the described LS spine changes.

Yes: Yes.  There are several options.  The gold standard is traditionally a decompression and fusion of one form or another.  There are many non surgical options as well.
