: Such a sprain of the ankle is as or more serious than some actual ankle fractures. To avoid ending up with chronic ankle problems for the rest of your life ( I've seen such patients) get an orthopedic surgeon with a special interest in complex ankle injuries to treat you. You may have to get one a the department of orthopedic surgery at a university hospital.

Yes: As long as you don't have bad arthritis in your knees, then there is no contraindications to operating a year after the injury.

Atropy: A herniated disc at the levels l4-5 and L5-s1 can produce atropy in the muscles that those nerves supply.  Typically in the anterior compartment of the lower leg and small muscles in the feet.
10 weeks of immobilization is quite a long time and was probably a factor in the atropy.

Tendonitis: For the ankle from trauma, ice and rest and 
Alive for pain 2x/d is helpful. Podiatrist can
be seen for wrap/boot as indicated/crutches
to take pressure off of ankle. These events
can take 6-8 weeks to heal and no running 
exercise/sports during the healing, stretching ok.

See your doctor.: Torn ankle ligaments are associated with severe pain, bruising, swelling, and difficulty walking. A torn ligament can often be diagnosed by a clinical exam that manipulates the ankle in certain directions. X-rays, mris, and ultrasounds can confirm the injury. Sprains and tears are treated differently. If not properly treated, a torn ligament can result in chronic pain and ankle instability.

See below : Clinical exam by an orthopedic surgeon is often sufficient.  Ultrasound is an alternative to MRI in selected cases. The nice bit about ultrasound is that it can image while stressing the joint. This is not the case with mri.

Yes: Your ankle may be a little less stable but fully functional. Also depends on how bad the tear.

Give it 24 hrs: If you can continue to weight bear without much pain, the chances are it is a simple sprain. However inflammation sets in about 6-12 hrs post injury and if you find that you are not able to put weight on it at any stage, see a Dr. You may need an X Ray to rule out an avulsion fracture, which is when a tiny flake of bone can get pulled off with the ligament during a sprain as well
