i have hep b for 2 months now and all my symptoms have gone yet my alt /ast numbers are steady rising. does that mean its going chronic? what can i do

You need a good doct: You may greatly benefit from seeing a liver specialist(hepatologist) or a gastroenterologist. They have the expertise to monitor you and guide you further. There is not much you can do except follow our doctors directions. They need to do further tresting to determine if your hepatitis is going to clear up on its own or it might become chrionic in which case you may need antiviral treatment

i am taking mtx for my sle (10mg/wk). my ast alt numbers have been rising (outside of range) since i started. at what point is it too high?

Your Dr decides: The decision on whether to continue Methotrexate in the face of some evidence of liver injury must be based on your rheumatologist's judgement. Remember that a few drinks a day can shoot the ast and alt up as well. Lupus is a rough disease, and the saying in lab medicine (my area) is "treat the person, not the number." be strong, stay proactive & best wishes.

i have had a  nephrectomy due to renal cell carcinoma in 2011. my cmp=  creatinine 1.72, gfr 31, alt 68, ast 45. are ^ numbers due to diabetes/kidneys?

Nephrectomy: ...will reduce kidney function in half resulting in increased serum creatinine (Cr) and reduced (GFR). With time the kidney compensates through hypertrophy, and these numbers partially normalize. Your numbers are not normal suggesting another cause of kidney dysfunction, possibly from diabetes, if there is a longstanding history of this. The increased AST and ALT suggest mild liver dysfunction.

can alt numbers be high and ast low?

Yes: Ast and alt are measures of liver enzymes and often are elevated in the setting of hepatitis or other causes of injury to liver cells. However sometimes one or the other will be much more elevated.  Sometimes fatty liver due to alcohol or some drugs/toxins can affect one enzyme more than the other.

alt 76 and ast 54.  are these numbers something to be concerned with?

Yes: I trust you do not overdo alcohol. One of the medicines you are taking may be bothering your liver, or less likely your skeletal muscles. Your physician knows your overall health and will perhaps wish to check your for hepatitis b, hepatitis c, hemochromatosis (don't miss this one), wilson's (maybe) & a few others. It may simply be a warning that a fitness-focused lifestyle would greatly help you.

what can cause alt to fluctuate? my numbers: august alt 71, november alt 59, current alt 79. all other bloodwork (including ast) is normal.

Depends: Elevated liver enzymes (alt and ast) can be due to a number of causes. Without knowing more information, it is difficult to answer. However, it can be due to fatty liver disease (very common now a days) or inflammation from other causes like viral, autoimmune, or metabolic causes. Discuss with your pcp or a liver specialist what the next steps are.

my liver count is ast 34 (normal),  alt 54 (high), ifcc 92 (normal),  g-gt 48 (normal).  are those numbers worrisome?

Doubt: UNLIKELY.  THOSE ARE MILD EELEVATIONS AND I DO NOT THINK INDICATIVE OF CIRRHOSIS.  IF YOU THINK THERE IS MORE OF A PROBLEM YOUR Doctor can order ultrasound.  An elevated ammonia and PT/INR .are associated with cirrhossis.

labs show ast 67 & alt 101.  everything else was normal.  are those numbers that high?  i am 38, no drugs or alcohol.

Liver function: AST and ALT are liver function tests, and the numbers you report are abnormal. There are several possible causes, and your doctor will likely recommend additional testing such as abdominal ultrasound and blood tests to try to establish the reason for the abnormality.

ast is 66 alt is 60  have appointment with gastro in 2 months.  i'm on a self imposed strict diet.  should those numbers worry me?

Not per se: All laboratory results need to be interpreted in the clinical context and the doctor who ordered the tests is usually in the best position to do that. Having said that, the enzyme levels are not too high for concern in the short term. Do talk to your doctor about all of your health issues and any medications, including, herbals, traditional Indian medications, at your next visit.
