Hepatitis: You may want to recheck it the levels.  If consistently elevated, you will need checked for chronic hepatitis virus infection, especially hepatitis b and c virus.

Repeat the test: Repeat the test now. If still abnormal, then the doctor will recommend work up. Normally, I would just wait a month and repeat the test, and maybe check for hepatitis. But why was this test done to begin with? Is she taking any medications?

Alcohol intake in your excess of 2 drinks per day for men is harmful to the body as it increases risk for liver disease, cancer, and heart disease.

14 year old son with elevated alkaline phosphate at 189. complained of fatigue. mono and thyroid test negative. should i be concerned?

Ok: Alkaline phosphate levels in children are often marked as HIGH as labs will use adult numbers as reference. This marker is present in liver, bones, muscles and intestines. As teens are in major growth of both bones and muscles they often have higher levels than adults. Without knowing the rest of the history or lab issues I don't think this lab value is a clue to the fatigue.

Rule out cancer : Without knowing your clinical background and history, when we see lab results of elev Alkaline phosphatase, abnormal liver function tests, we are concerned about inflammation of the liver, from many different causes: infection, alcohol hepatitis, liver failure, and even liver cancer.  Your dr must be concerned about looking for lesions in the liver if she ordered an MRI. Ask her directly.

Stay with your MD: Repeated mild Acute pancreatitis due to Inflammation, Infection, or other causes like oral Contraceptives/ medications, can, over time cause pancreatic scar tissue formation; aka Chronic pancreatitis which is also caused by chronic Alcohol use, Obstructive Liver/ pancreas diseases + more can all cause elevated alkaline phosphatase. Need to stay under the care of your MD to navigate to wellness.

Misunderstanding : The number 275.42 is the diagnosis code for hypercalcemia (high calcium). That is found on blood tests not an ultrasound. An elevated alkaline phosphatase can come from liver or bone. You need to ask your doc if the rest of your liver tests are normal.

Liver irritation: These numbers mean that you have sustained some sort of liver irritation. You need to be tested for viral forms of hepatitis. Until you know the cause of these elevations you need to avoid all alcohol and acetaminophen. See your doctor soon for further testing to determine the cause.

How high?: Do you have any symptoms? Why was the test done? All laboratory results need to be interpreted in the clinical context and the doctor who ordered the tests is usually in the best position to do that. Having said that, high alkaline phosphate may due to liver or bone disease, and sometimes intestinal disorders. It is normally high in children and during pregnancy.
For good health - Have a diet rich in fresh vegetables, fruits, whole grains, milk and milk products, nuts, beans, legumes, lentils and small amounts of lean meats. Avoid saturated fats. Drink enough water daily, so that your urine is mostly colorless. Exercise at least 150 minutes/week and increase the intensity of exercise gradually. Do not use tobacco, alcohol, weed or street drugs in any form. 
Practice safe sex, if you have sex.
