currently sched. for a da vinci pt. hysterectomy. 38y, healthy. endometriosis, adhesions. very concerned about complications. is this the best method?

Talk to your surgeon: You need to have a "sit down" discussion (Consultation) with your GYN surgeon. The method of hysterectomy is dependent upon to nature of the "disease" to be removed, the surgeon's experience, the hospital facilities/equipment with available anesthesia,  risk of complications, and most importantly, your level of comfort with them all. You will be asked to sign an Operative Permit at surgery.

i've had 2 c-sections and a hysterectomy.  i have adhesions. when i have a colonoscopy they've had to use a pedi scope.this last time pedi scope wouldn't pass thru. so used endoscope.need to worry?

Hard to say: The chances are that because of the adhesions you may have abdominal and/or pelvic pain on and off. If the endoscopy went well and no serious pathology was found other than the adhesions you should have no problems other than pain to one degree or another. Adhesions do not go away.  Keep in close contact with your physicians because of your cardiac situation.That has no connection to your surgery.

I would avoid it: And would actually recommend a product that has both estrogen and progesterone, but speak to your doctor about this first. The concern lies from some theoretic concerns that if there are residual endometriotic lesions left after the surgery, the added effects of both Progesterone and estrogen would prevent any malignant transformation.

Contact your surgeon: Follow-up of any surgery is part of the procedure. Contact the dr and ask for information and an exam to assure that healing is progressing as expected. No one else can address your case as well as those already involved. During this time avoid alcohol and tobacco. Cells heal much better w/out irritants like alcohol and tobacco, etc. Be sure to get help. Keep asking till ok.

Previous scarring: Previous surgical scar tissue (adhesions) make this a more complicated question to answer because the subject ovary may be "stuck" to other tissues like bowel, pelvic sidewall or bladder. If you don't want to return to surgery after this operation you might give consideration to hysterectomy with ovarian removal.  You will need a very competent surgeon to undertake this operation- interview wisely

Endometriosis?: Did you have endometriosis? It's possible you could have it in the vaginal cuff. It could also be a tubal prolapse. Either way. You should probably have your ovaries removed. Which could be done laparoscopically.

how to prevent adhesions after a total abdominal hysterectomy?

Difficult: When ever there is a scar , what ever the reason , surgery , or infections adhesions will form even from a tinny  scar , can&#x27;t be prevented but could be minimized , by tension free closure , minimum dissections , if possible retroperitoneial ( out side ) approach, leaving   with minimum blood ; secretions inside  peritonaeum. Best prevention is if possible avoiding surgery.

The severity ...: of your post-operative swelling warrants a call to your surgeon, who knows exactly what procedure was performed and with what findings. We can only guess, but would urge that you don't ignore the problem.
