: "Restricted diffusion on DWI" and "hypointense on ADC" refer to techniques of assessing abnormal perfusion in areas of the brain during MRI studies.  Either or both can refer to tumors or damage secondary to stroke effects.  Consult your physician or the radiologist reading the MRI for more specific clinical correlation with your MRI findings.

Stroke : Not to worry. Perivascular spaces (Virchow-Robin spaces) are normal and not caused by a stroke. They can occur throughout the brain. The perivascular spaces vary in size (up to 5 mm) and are associated with arteries in the brain.

4 months post pontine stroke in a previously healthy 44yr old male. orgasm causes severe neck pain, nausea and dizziness lasting 2-4 hours.

Bothersome: The major question is the cause of your stroke, as there is risk of another(unless it was of traumatic origin, such as vertebral dissection).  Your description refers to a migraine-like reaction, which may be due to neurochemical glutamine sensitivity in the area of damage, and perhaps a migraine preventative will quell the reaction.  But get this totally defined, even a second opinion.

my husband suffered from brain stroke in pons area one and a half year back..he is recovering but is unable to walk as his left side is still weak.

you say he is recovering  and left side is weak which is not unusual after stroke
he may benefit from physical therapy, discuss with his doctor.
I am sorry we can not advice more than that we do not know the details about his other coexisting conditions. Needs to control hi bp, cholestero, sugar if he has diabetes&take meds for secondary preve.

Follow up: Follow up with doc is needed. Many meds are to be taken indefinitely for strokes, while others for limited time. Follow up will likely be needed with a neurologist as well, they all may be captured in discharge papers.

Clot busters: If a stroke patient can get to the emergency room within 4 hours, tpa (alteplase) can be given by vein in hopes of breaking up the clot blocking the brain artery.  If the patient gets there before 6 to 8 hours, we can place a catheter within the artery in the brain and frequently be able to break up the clot that way.  It is riskier, however.

New onset: An acute ischemic stroke is one whose onset was within the past 24 hours or less.  An ischemic stroke refers to brain damage due to an obstructed artery, whereas a hemorrhagic stroke is one in which brain damage occurs as the result of bleeding  (hemorrhage).

: Most commonly, the local bleeding commences following a thrombotic or embolic block of a blood vessel, with the distal damaged area subsequently developing a hemorrhagic outcome.  This is too variable to make any reasonable comment about the speed of development, but must always be considered in a deteriorating stroke victim.

Stroke: Encephalitis or brain inflammation causes various neurological signs and symptoms and abnormal lp fluid  including dementia, but I'm not familiar with the term inflammatory stroke. Strictly speaking there are 3 types of stroke-hemorrhagic, embolic and ischemic.
