Possibly: Acute? Chronic? Recent repair? Nausea? Skin turning red?  Previously pain free?  Sounds like you need to be checked sooner rather than later.

Always a risk: Abdominal wall defects are always a risk for incarceration & strangulation.First if you are over weight , reduce your weight, ware an abdominal binder, try to eat small meals  and don't get constipated, if these measures fail and you no choice but surgical repair.

See your surgeon: It is not clear to me from your comment where your incisional hernia was located or where the bulge is now. Incisional hernias can reoccur. See your surgeon to evaluate your problem.

had incisional hernia abdominal wall repair w mesh 2018 after open gallbladder surgery 2016. i have pain, stiffness, heaviness ever since. i have pain, felt hardness from coughing while sleeping a few times. has it recurred? what tests can be done?

Many: Abdominal pain of sudden onset, severe, associated with  Bleeding,fever chills vomiting weight loss or dehydration requires urgent medical evaluation to check for emergency type issues. Like appendicitis GI bleeding diverticulitis cancer 
You need to get specific advice from your physician or a physician in person testing can include CAT scans MRIs ultrasound lab work

i had an incisional hernia repair, lower right abdomen 4 weeks ago. i am experiencing pain over the area, nausea and diarrhea. a ct scan of the abdomen reveals no abnormalities or causes for the symptoms. is there anything i should do for this?

Hernia: It is reassuring the CT was normal. Have you had the surgeon examin the area since it is tender? The nause and diarrhea could be unrelated and due to a viral infection. If you are able to keep food down, stay hydrated and symptoms are improving then I would hold off on any further work up and see how things go.

Possibly: If you know you have an elevated WBC you must be in contact with medical care source.  If you also have pain, a bulge, and nausea, you could have an infection or obstruction, either of which could be emergent.  If you have a source of care, go back to them and demand an explanation for the elevated WBC and pain.  Ideally go back to the same people who diagnosed the abscess and removed mesh, O/W ED

Hernia: It is a hernia, or protrusion of abdominal contents through a previous laparoscopic surgery access site. They can and should be repaired if symptomatic, as there is risk of incarceration (getting stuck) in these small hernias.

He may: ...Want to correct the defect and would like to lose the wait to properly assess the condition as well do a better on you. If i would suffer from pain i would certainly try to lose the weight to help him help you!
