Yes: I would suggest you bring your child to see a local pediatrician to be evaluated. Since your child is only 4 months and has terrible symptoms like you described, there are not many otc meds you can use.

Acetaminophen infant: Give her Acetaminophen infant for fever around the clock ( follow the instruction ), cold compress to reduce her fever. Make sure she is still drinking . Call and bring her to pediatrician tomorrow. No need for er at this time, as long as she is awake, playful and drinking well. If she is lethargic- absolutely bring her to er.

Nasal saline: The use of liberal amounts (think garden hose) of nasal saline can help thin out thickened secretions & allow their removal with suction or by draining back into the stomach.This can also remove dust particles or other aggravating agents that make baby stuffy.

Maybe: Try clearing her nose with your bulb syringe, and using either a humidifier or take her into the bathroom, turn the shower on as hot as it will go, and let her breathe in the steam.  If not better, see your doctor.

URI: A plain uncomplicated cold may last up to 7 to 10 days, the mucus changes from clear to cloudy and you may have a productive cough. Make sure you run a humidifier and suction the nose out with saline drops. If your child seems to be in any way worsening, has difficulty breathing and has cha ge in behavior and activity, is not eating or drinking and has a protracted fever, see your child's doc asap.

Cold: Cool mist vaporizer/humidifier can often help. Saline nose drops followed by bulb suction can serve as a decongestant. Over-the-counter cold/cough remedies are not recommended for this age group. If at any point he develops a fever or his appetite decreases, i recommend consulting with your doctor.

Cold: it is better not to give your child any medications. Try suctioning the nose, elevation of head of the bed and run a humidifier.
