sm antibod 30-40%.

About 1.5 million americans have lupus, and 1/3 will develop kidney involvement characterized by protein in the urine and elevated blood creatinine levels. Trace leukocytes in the urine by itself and unassociated with urinary symptoms is most often normal finding.

None: lupus needs to be controlled with meds that treat the underlying inflammatory process, not with pain meds.

Not likely: Why worry about 3 disorders, as you likely possess one unifying diagnosis to explain your problems.  A good place to start is with an endocrinologist to deal with diabetes and the complications secondary to this.  Focus on these issues, and later, if things seem peculiar look into co-morbities.  Lastly, if you possess diabetes and lupus, doubt you have ms in addition.

Rheumatologic issues: The specific diagnosis of lupus wouldn't run in families as much as the tendency towards rheumatologic conditions in general - that tendency runs in families - lupus, rheumatoid arthritis, Sjögren's syndrome, scleroderma.  Can find all of these in family lineages.  Have a good history/physical exam/blood testing coordinated by Rheumatologist.

i have an ana that goes from 1.1 or below up to 1.3. does a fluctuating result like this mean i don't really have lupus?

Not really: ANA results are given as titers. A titer of less than 1:40 is considered normal.  Many people in the general population will have low titers of ANA (1:1 or 1:2) and will not have an autoimmune disease.  95% of people with lupus will have an elevated ANA test.  If your ANA test is negative or low; your chance of having lupus is less than 5% (but not zero).  A diagnosis of lupus requires other tests.
