Inguinal Hernia: It is most likely a hernia. But regardless, he should have his family physician take a look and decide what he needs for treatment.

Probably neither: If the mass is in the skin, it is most likely a skin cyst, a larger mass could be a hernia. Needs to have evaluated by a physician.
