Normal: After groin hernia repair it is normal to have painful and/or swollen testes and scrotum. This is because the testicles are hanging from a cord that is in the operative field, and is involved in the operation. It should get better in 1-4 weeks depending on many factors including technique or repair. Laparoscopic repair is generally less time.

Testes hurt : During inguinal hernia repairs, the spermatic cord is pulled on a bit and that is connected to your testicles. So, sometimes you can have swelling, bruising, or pain in your testicles after surgery.

Vascular engorgement: The hernia surgery tightens up the inguinal canal and some swelling can occur as a result of the surgery and therefore some venous engorgement which could cause the testicular aching and scrotal pain.
