Rectal Pain: You need to tell doctor of your pain so that he or she understands your condition more. Are you doing everything you were advised to do at your doctor visit. Has the doctor seen the condition recently. There are several different medications for inflamitory pain in this area from steroids, anti fungals, anti-inflamitory (nsaids). Call your treating doctor now, and give accurate update.

Anal fissure: You need a cream for the fungal infection and a topical or injected treatment for the fissure like a nitrate or a CCB or even Botox . This can be best administered by a general or colorectal surgeon sometimes pain medications are needed in the short term along with stool softners.
