See MD: Most likely hemorrhoidal, but could be a perianal abscess (pus pocket-needs drainage and antibiotics), other gut problems, e.g. Fistulas (abnormal connections internally). Less likely gyn relaterd. In any event, need to see md, get examined and treated. Fever? Altered bowel habits? Get seen asap.
