Sometimes: Surgery for inguinal hernia in a male may sometimes affect fertility; it often involves antibody to sperm. Howver, many men with hernia repair suffer no effects. First a semen analysis to determine if sperm are abnormal; if not an antisperm antibody test might be useful.

Maybe...: During hernia repair, particularly in young boys, the vas deferens (tube that carries sperm) can be damaged and scar, preventing sperm passage. Rarely, in adults having this operation, the testiculat blood supply can be compromised and cause shrinkage or even loss of a testicle, which can affect fertility. But keep in mind, most hernia repairs go well without any problems.
