By far the most common cause of penile blister-like sores, especially with redness, is genital herpes. You do not have to be sexually active recently to have herpes: if you have ever had sex, and with just a single lesion, if it's herpes probably it is a recurrent outbreak of infection acquired months or years previously. Get seen ASAP: accurate diagnosis of herpes requires test within 2-3 days.
