If syphilis is confirmed, follow the advice of your doctor or clinic. Syphilis usually is not transmissible by sex after a year, but there are exceptions; the safest thing is to not have sex with your fiancee (or wife) until you have been treated. Syphilis more than a year in duration usually needs 3 doses of long acting penicillin, but you will be non-infectious 24 hours after the first dose.
