It is hard to say whether acute epididymitis is caused by infection alone as there is no way to take a culture or other ways to prove it but since it responds to antibiotics they are used. It can be caused by trauma too

Most epididymitis is caused by infection, and at your age STIs are by far the most common causes, especially chlamydia and gonorrhea. Injury to a testicle can cause similar symptoms, but probably not epididymitis itself. Physical "strain" from heavy lifting was once thought to be a cause, but rarely if ever. Non-STI urinary infections do it, but mostly in men over 30.
