Post-circumcision swelling in a 4-month-old should settle down in about 5 - 7 days after surgery. You may not know what technique was used. Did the surgeonuse sutures? or was circumcision clamp used or PlastiBell or other circumcision deviceused.Moderate to severe swelling can occur if there is and bleeding under the skin, this can take longer for swelling to go if it occurs. Try to consult surgeon

Also, (believe it or not) raw organic honey applied to the affected foreskin area twice a day works as a natural antibiotic. Of course, good hygiene ( and that does not mean antibacterial soap) washing after each urination till resolved is important.
