How long can a person safely use otc 1% hydrocortisone cream to relieve itching on an anal fissure?

As long as needed: Although it is advertised as maximum strength it is actually one of the mildest steroid creams available. That site is often prone to an overgrowth of yeast which the cream does not effect. Using a clotrimazole cream occasionally can reduce this yeast.
