Many causes: Scrotal swelling has many potential causes. Enlargement of the testicle itself is concerning for tumor. Fluid can accumulate around testicle - hydrocele or spermatocele. A groin hernia can extend into the scrotal sac. Infection adjacent to testicle will cause painful swelling and tenderness. Finally, varicose veins above the testicle (varicocele) causes swelling above the testicle.
