Perianal fistula?: I would worry about an underlying perianal fistula, especially for recurrence near the anus and perhaps also the groin. A fistula is a defect in the rectum burrowing toward the skin. If so, more extensive surgery may be required to remove it. You also may need evaluation for underlying problems like inflammatory bowel disease. Ask your doctor about it.
