It's complicated : I assume you want a work up for a problem with infertility. You should really be worked up with your partner, not on your own. You should spend some time on the Web to see what is involved in a full work up. Good site is www.mayoclinic.com search term would be infertility work up. Then go see a doc who specializes in infertility.
