Very common: Chronic prostatitis (CP) and chronic pelvic pain syndrome (CPPS) may be the same, as you seem to know. Very common. Inflammation sometimes is present, e.g. white blood cells in prostate fluid or urine. I can't judge if past prostate injury is contributing. CP/CPPS pain often is felt in testicles, but no actual testicular problem. It's not known to affect fertility. Keep working w/ your urologist.
