See a doctor: Coughing up blood can be a sign of a serious problem. A chest x-ray can rule out most of the bad things.

Smoking: Irritates tissues and can cause oral, throat, and/or lung cancer. If you have gum disease, smoking can exacerbate the gum destruction. Im portent that you see bot a Dentist and a Physician immediately. Report all symptoms. And quit smoking.
