HSV and pilar cysts: The difference is that HSV blisters are painful, located on the skin and will be present during the outbreak and then the blisters will disappear completely once the outbreak has resolved. Pilar cysts are firm, slow growing subcutaneous nodules that do not spontaneously disappear. They are painless, unless they become infected and are most commonly located on the scalp
