Check with GI doc: First question is his age and has he had a colonoscopy. Pain can be from hemorrhoids or something more serious so I would check in with doctor, or gastroenterologist if he has one.

See a DR: The only chance for an explanation will come from an exam. Best DR for this is gastroenterologist. Probably a local rectal condition which will be remedied after it is identified.
