Discuss it NOW....: Discuss your history of severe abuse with your ob, and of course mention your narrow hips. I would think that a sensitive physician would understand why you may not want to go through a vaginal birth, especially one that could possibly be more prolonged than normal. Don't wait 'til the last minute to discuss this. You want to get his understanding if your situation now, so this can be planned.

Depends: Some insurers require you to go through a trial of labor or they may not cover the extras required for a c-section delivery. Since most of healthcare is now controlled by the insurance industry you may not get to preschedule one unless you pay out of pocket. Discuss this early with the OB, if there is a way they can do it many will try.
