Using condom: Will prevent semen from entering vagina at time of ejaculation (Seminal discharge). Withdrawing penis just prior to ejaculation does not prevent sperm from entry because of pre-cum which escapes before ejaculation & may contain sperm.

Hematospermia: A small amount of blood in your semen. Can be related to trauma, infection or illness. I recommend a consultation with a urologist and a prostate examination with blood tests for prostate illness as well as a urine analysis.
