Could be either: Both could explain an improvement in symptoms. An acutely herniated disc has swelling and blood associated with it which can resolve to some extent, whereas cortisone gives a booster dose of anti-inflammatory which reduces the swelling and inflammation if the disc and the inflamed nerve.
