Probably nothing: A blood clot under a nail is usually a small circle of dark blood which is painless, but unsightly. No treatment is needed as it will grow out and go away.. In an emergency room a doctor may "drain it". That involves heating a paper clip and burning a small hole in the nail - completely painless. The blood clot drains completely and the hole in the nail grows out in a month or so. Peter wilk, md.
