Always dimensional: All personality syndromes exist on a continuum of severity, from mild through moderate to severe. For example, you can be a little narcissistic or extremely narcissistic. At some point on the continuum, the person is impaired enough to get diagnosed with a disorder, but it is really just an arbitrary point on a continuum.
